The referral form is used by physicians to order a home sleep test.
You do not need a referral form to have a test performed if you want to pay cash. However, a completed/signed referral form (and face to face clinical notes) will be required by your insurance company if you plan to use insurance to cover the cost of treatment for your obstructive sleep apnea or if you want to apply your out of pocket cost for the apnea test to your deductible. If you suspect you may have sleep apnea and plan to use insurance to pay for your therapy, print a copy of our referral form and take it to your doctor. Your physician completes and signs the form and the physician (or you) can fax or email it to us. We do the rest, it's just that easy. |
What if you don't plan on using insurance to pay for therapy if needed? Then you don't need a physicians order or referral to perform a home sleep test.
Can you use your insurance to cover the cost of the home sleep test? We are currently in network with Medicare, Tricare South and numerous other insurance companies in the state of South Carolina. We do not accept insurance outside of the state of South Carolina. If you pay cash can you still apply it to your deductible? Absolutely! Any cash expenditure for healthcare can be applied to your deductible, your insurance company will supply you with a form to complete and submit a copy of your receipt. If I have a high deductible would it be better to pay cash for my pap therapy? Absolutely! The cash purchase of pap therapy is significantly lower than what you are billed when using insurance. Typically the cash price is 50% lower than when using insurance. And remember, any cash expenditure for healthcare can be applied to your deductible |
